Membership Form
Membership in the Lay Faculty Association (LFA) Local 1261 is a voting
status held by full dues paying individuals on the faculty of a school
represented by the LFA, who are eligible for the rights and benefits
negotiated by the LFA and provided under the Collective Bargaining
Agreement, which may include but are not limited to the LIUNA pension
plan and medical benefits provided by Mason Tenders District Council, and
where applicable, any other LFA benefits the Board of Directors authorizes
under the LFA Constitution and By-Laws or policies passed by the regular
membership in the LFA Local 1261
Associate Application Forms
Associate status is a non-voting category in the Lay Faculty Association
(LFA), Local 1261 for staff, custodial, and administrative staff in the schools
the LFA represents. The fees paid may be for the benefits the LFA has
secured for the class of membership, viz., LIUNA pension and Mason
Tenders Welfare Benefits, and where applicable, any other LFA benefits the
Board of Directors authorizes under the LFA Constitution and By-Laws or
policies passed by the regular membership in the LFA Local 1261
Special Associate Application Form
Special Associate status is a non-voting category of non-unit member, i.e.,
any coach, moderator, coordinator, summer school teacher, or activity
leader that the LFA negotiates a salary or stipend for said activity during
the school year in a school that the LFA represents. The fees paid are for
negotiating said salary/stipend and any benefits that the LFA Board of
Directors may secure for this class of membership under the LFA
Constitution and By-Laws or policies passed by the regular membership in
the LFA Local 1261
Member Update Form
This form should be filled out by any dues/fees paying member or
associate who is employed in our eleven schools. Use this card for any
name (including marriage and divorce) changes, contact information
amendments (including email), or change in job status. If you have any
questions, please ask your school rep or call/email the LFA office.
Hard copies can be found with the school delegate or alternate delegate
and at the LFA office, and should be returned to the school reps or mailed
directly to the LFA office so that updates can be made in the LFA and Liuna
databases. Please also make sure to update your school's business office
with these changes.
*This form alone does not cover changes to the Mason Tenders District
Council database. Please see the MTDC form.
Travel & Hours Reimbursement Form
This form is for any and all members, board members, and executive
board members who travel or do extra work for the union (serving on a
committee, coming to a meeting, etc).
Monthly Reimbursement Voucher:
This form is for Executive Board and Board Members who need to be
reimbursed for any expenses they lay out.
Mason Tenders District Council Update Form
This form is for any and all members and associates who arecovered by
Mason Tenders District Council’s health benefits and need to file a change of
address with their office.
* Mail it directly to the address on the form. A blue card should also be filled out
with your school delegate or alternate delegate and submitted to the LFA so that
our files and the files for LiUNA in Washington, DC can also be updated.
Audit Committee Travel & Hours Reimbursement Form
This form is for audit committee members to apply for reimbursement for hours
and mileage
