IMPORTANT MESSAGES REGARDING THE MASON TENDERS

DISTRICT COUNCIL WEFARE FUND:

*UPDATE OCTOBER 2020:

This Notice describes important changes to the Mason Tenders' District Council Welfare Fund (the "Fund") for members of Lay Faculty Association Local 1261. Please take the time to read this Notice carefully and keep it with your copy of the Fund's Summary Plan Description ("SPD"). RETIREE CONTRIBUTION INCREASES EFFECTIVE JANUARY 1, 2021 AND NEW MEDICARE ADVANTAGE PLAN OPTION: As you were previously advised, the amount that retirees are required to pay for the Fund's retiree coverage will increase as of January 1, 2021. The new contribution rates for January 1, 2021 and July 1, 2021 are set forth below in this Notice. The rates for January 1, 2022 will be communicated closer to that date based on the cost of coverage at that time. This Notice is also to advise you that the Fund is offering a new lower cost benefit option for Medicare-eligible retirees and spouses, as an alternative to the Fund's current retiree coverage option for Local 1261 retirees. Medicare-eligible retirees (and/or spouses) may elect either of these options. The Medicare Advantage Plan is an Aetna-insured plan for medical and prescription drug benefits (as described further below). If you (and/or your spouse) elect the Fund's Medicare Advantage Plan option, you will still be a Fund participant and receive the other Fund benefits to which you are entitled as a covered retiree and spouse (i.e., dental, vision and death benefits). Enclosed is an Election Form for electing either the Fund's current retiree coverage or the Fund's Medicare Advantage Plan for medical and prescription drug coverage (with the other Fund benefits), effective as of January 1, 2021. Please follow the instructions in the Election Form, and be aware of the November 20, 2020 deadline for your election and the end-of-the-month deadline for monthly payments. Please contact the Welfare Fund directly for official information at: 1.212.452.9700. The Welfare Fund is responsible to answer your questions and to hear your complaints.
